Each team member is required to have a completed health form and publicity waiver.
Coaches - please download these files, run copies for each student, collect them prior to your first tournament.

You will need to hand in a copy of the Publicity Waiver and Roster/Code of Ethics form at both regions and state tournaments.

You will need to have the health forms for each student with you at each tournament you attend.
There is a spot on your team roster form verifying that you have these forms with you.
